    • Read the agreement, pages 1-23.
    • Complete the Enrollment form, pages 24-27.
     

    Mail form

    • Make a copy of the agreement for your records

    Getting started (account setup)

    • Account setup takes two business days from date received.
    • Customer Service emails the Primary Company Administrator initial instructions.
    • Administrators receive login instructions via email.
    • Customer Service mails executed copies of the agreement to customer.
